I declare under my own responsibility, with full knowledge of the consequences of making a false declaration, that the information provided in this form is accurate and contains no misrepresentation or concealment of facts relating to the insured event, and that I am aware that any false declaration may result in criminal liability under the law and in my exclusion from any further procedure.

By signing this claim for compensation, I declare that I freely and fully consent to the processing of my personal data to the extent necessary pursuant to the law on insurance and reinsurance, the laws and other acts regulating insurance activities, and reinsurance agreements, in accordance with the law on the protection of personal data and the company’s privacy policies.

Note!

Pursuant to Law No. 32, dated 16.03.2021, “On Mandatory Insurance in the Transport Sector”, the following is provided:

Article 10

1. “The responsible insurer is required to process the claim for compensation within the following time limits:

a) for bodily injury claims, within 90 (ninety) calendar days from the date the claim for compensation is submitted;

b) for property damage claims, within 30 (thirty) calendar days from the date the claim for compensation is submitted.

2. For the purposes of paragraph 1 of this Article, the date of receipt of the claim for compensation shall be deemed to be the date on which the claim is submitted together with the complete supporting documentation.

This note does not apply in cases where customers request the repair of damaged motor vehicles at repair shops.

Please be advised that submitting false documents or misrepresenting circumstances for the purpose of unlawfully obtaining insurance benefits constitutes a criminal offence under Article 145 of the Criminal Code and is punishable by a fine or imprisonment of up to 5 years. Making a false declaration may result in criminal liability under the law and in your exclusion from any further procedure.
